2. Open the PDF and type inside it to fill in the form.
3. Print out the completed form.
4. Sign the form.
5. Have your Pastor sign your form as well.
6. Mail form back (address below), or scan / email it back (click here for email).
7. Payment can be by check included with the form, or pay online (click here).
